The Cathedral of the Nativity is the main cathedral of the Yuzhno-Sakhalinsk diocese in the very center of the city, accommodating up to a thousand parishioners at the same time.

The temple is based on a five-tiered iconostasis, including 98 icons, the main entrance, altar and stylobate are covered with magnificent mosaics with a total area of 175 m², and sunlight illuminates the interior through more than 200 m² of stained glass windows. The main lighting is provided by a two-tiered choir in the shape of a dome. In the center of the temple there is a Florentine mosaic of 10 m² with the image of the Star of Bethlehem, which is fully visible from the fourth floor of the galleries.

The first Divine Liturgy in the lower church in the name of the Three Saints was held on June 7, 2013, and in the upper church of the Nativity on August 14, 2015.
